This position is operationally managed within the Complex Developmental Behavioural Conditions services at Sunny Hill Health Centre for Children.


Working from a family-centered, trauma-informed care model, the Care Coordinator works without general supervision as part of multidisciplinary community and hospital teams and alongside clients, to provide clinical and consultative social work services to families whose children have complex care needs.  Working in close communication with broader community-based service providers and relevant medical staff across BC as needed, the Care Coordinator performs professional social work services with a focus on advocating for clients and supporting coordination of services as close to home as possible. The care coordinator engages in on-going service planning, program development, evaluation and quality improvement activities to achieve excellence in client care and also supports capacity building among patients and families, hospital services and community partners. The Care Coordinator utilizes advanced psycho-social assessments to identify the patient and family needs and goals and to support the coordination of a Collaborative Plan of Care.  The Care Coordinator provides advocacy and consultation to team members and relevant community agencies and provides leadership, direction and support to the inter-professional treatment team around matters concerning community and hospital resources.  The Care Coordinator plays a central role in liaison, and applies advanced knowledge, skill and clinical experience in facilitating communication and decision making around patient care. The Care Coordinator promotes family-centered care within BC Children’s Hospital and Sunny Hill Centre for Children, and the community.


Example of Duties/Accountabilities

  • Using advanced knowledge and expertise, functions in a lead role as Care Coordinator for the patient and their family as the point person for the patient/family, BCCH/SHHC clinics, and for the involved community providers.  This includes: providing advanced level intake and family needs assessment, making recommendations on service planning/delivery throughout BCCH/SHHC, providing clinical leadership to the involved interdisciplinary clinicians and determining when and how to integrate with other community resources over time.
  • Provides advanced clinical consultation on potential patient referrals and participates in complex case planning for children and youth with multiple and complex developmental and behavioral needs, using methods such as phone or in person consultation to clinicians/health providers at BCCH/SHHC and throughout the province. Assists with coordinating follow-up with BCCH/SHHC clinics and community partners; utilizes teleconferencing to include health care professionals in care planning alongside patients and families around the province.
  • Conducts a comprehensive review of BCCH/SHHC patient chart to reduce repetitive parent/caregiver information and history.  Using a family centered care framework, conducts an intake assessment by phone or in person, which is intentionally parent/caregiver driven and guided by their perspective and needs. Obtains additional relevant psycho-social information; for example, the patient’s family, social supports, environment, financial status, community resources, gaps in service, etc.   Initiates a plan of care which includes the goals and needs identified by parent/caregiver, and other areas of agreed support suggested by comprehensive intake assessment.
  • Advocates for the patient/family within BCCH/SHHC to assist in streamlined service delivery. Can include; communicating with Program Managers/Directors, Physicians, Case Managers, Clerical staff to plan innovative pathways, efficient timelines for services/assessments, a decrease in paperwork/forms, coordination of appointment bookings to reduce long distance travel, and work with clinicians to share relevant information for purposes of collaboration in patient care. 
  • Establishes and maintains effective collaborative and constructive liaison relationships with a variety of individuals and groups, including patients and families, community primary care physicians and pediatricians, community mental health providers, MCFD social workers, schools, hospitals and other agencies, in order to coordinate services.
  • Provides consultative and direct services to patients/families and staff regarding availability and access to community resources to facilitate planning. Aids patient/family access to community services by methods such as providing resource information, contacting relevant programs/services/agencies, and facilitating practical supports (i.e.: referral letters, funding forms, transportation assistance).



Education and Experience

  • Master’s Degree in Social Work from an accredited School of Social Work, including relevant graduate level mental health courses.
  • Minimum 5 years’ experience specifically providing assessment, coaching and care coordination both within Hospital outpatient settings and in the community, with families of children with complex behavioural and developmental conditions or equivalent combination of education, training and experience.
  • Current full registration with the British Columbia College of Social Workers.

  • Demonstrated ability to establish rapport with clients and families in order to explore issues that are beyond the initial presented concerns.
