Job Summary:

Reporting to the Clinical Coordinator, Medication Reconciliation or designate, the Medication Reconciliation Pharmacist provides clinical care to oncology patients through the completion of medication reconciliation, patient medication counseling, and consultation with physicians, nurses and other health care providers. Additionally, the Medication Reconciliation Pharmacist acts as a department resource, participates in medication use evaluations and provides instruction to pharmacy students and residents.
Duties/Accountabilities:
1. Provides clinical care to oncology patients through the completion of medication reconciliation processes, patient medication counseling and consultation with physicians, nurses and other health care providers. 2. Provides instruction and/or supervision to pharmacy students and residents by assisting with educational material, demonstrating procedures, supervising during patient care activities and providing input into performance evaluations. 3. Conducts medical literature research, critically evaluates and interprets clinical trials data, seeks interdisciplinary input as appropriate, and summarizes and reports information. 4. Participates in quality assurance checks as required to ensure relevance, appropriateness, and timeliness of program delivery. Recommends program improvements as required. 5. Maintains current knowledge in Pharmacy by methods such as reviewing literature, consultation with other health care professionals, contributing to pharmacy publications such as the Systemic Therapy Update and the Cancer Drug Manual. 6. Performs technical duties, chart reviews, patient assessments, creation of individualized therapeutic plans, monitoring patients for desired therapeutic outcomes, and collaborates with physicians and other health care providers to ensure safe, effective, and patient-centered care. 7. Attends and presents at relevant education programs to contribute to the continuing improvement of care in Oncology Pharmacy Services. Represents Pharmacy on regional and provincial committees. 8. Performs other related duties as required.
Qualifications:
Education, Training and Experience
• Current registration as a Pharmacist with the College of Pharmacists of British Columbia. • Completion of an accredited Hospital Pharmacy Residency Program. • Minimum two (2) years of recent related oncology and/or hematology experience, and/or an equivalent combination of education, training and experience.
Skills and Abilities
• Broad knowledge of BPMH, medication reconciliation, medication safety principles and practices. • Knowledge of evidence-based clinical pharmacy practice. • Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision. • Ability to communicate effectively both verbally and in writing. • Demonstrated ability to instruct staff and students, including clinical training and didactic instruction. • Knowledge of hospital pharmacy computer applications, information systems, patient care information systems and related software. • Knowledge of drug distribution processes in a hospital setting. • Ability to establish and maintain effective working relationships with other health practitioners, as well as patients and family members. • Ability to organize and prioritize work. • Ability to operate related equipment. • Physical ability to perform the duties of the position.
