Job Summary:
In accordance with the Purpose, Vision, Values and Coast Salish Teachings, and strategic direction of PHSA, safety, including both patient and employee safety, is a priority and a responsibility shared by everyone at PHSA. As such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.
Serves as the primary resource to staff and other health care professionals for the care of transplant patients and their families. Develops patient teaching standards and programs using a specialized theory and practice base. Facilitates and coordinates the education and support for staff and patients/families. Manages the operation of the clinic and supervises clerical staff. This position serves as a patient advocate, care facilitator and clinical nursing resource.
Duties/Accountabilities:
Duties
1.Responds to referrals from nursing staff and other health professionals related to the care of solid organ transplant patients and families. Uses a specialized theory base to provide direction and support for the development of care plans and teaching plans and assists in solving difficult patient care issues.
2.Coordinates the overall nursing care of solid organ transplant patients and families. Follows patient progress from pre-transplant period to post-transplant and provides information to the transplant team concerning current patient status during transplant rounds and on an ongoing basis.
3.Analyzes data from a variety of sources to identify quality assurance issues and provide recommendations for the resolution of such issues.
4.Manages the operation of the Clinic by providing nursing services and supervising the clerical staff.
5.Books or assists families to book investigation procedures with labs and hospital departments and referring specialists. Receives and reviews test results.
6.Develops patient teaching standards and programs based on the analysis of patient needs and current relevant theory, utilizing appropriate community and regional resources.
7.Assesses patient and family needs using advanced physical assessment skills and interview skills. Plans and provides support for patients and families in collaboration with the Transplant team. Assists patients and families with problem solving.
8.Makes arrangements for out-of-province referrals (i.e. extra renal transplants) and living donor transplantation and maintains current information exchange with out-of-province transplant centers.
9.Coordinates activities associated with the care of the recipient at the time of transplant when this occurs during working hours.
10.Provides recommendations to the Patient Services Director concerning the most efficient use of budget and resources.
11.Provides input into planning the development of regionalized clinical transplant services in collaboration with the BCTS. Provides input into the development of education programs and other initiatives of the Society.
12.Identifies ethical issues in transplantation and participates with the team in their resolution.
13.Collects data for research projects as directed by BCTS (such as clinical trials). Collects program statistics for transplant patients.
14.Participates as a member on multidisciplinary committees; attends meetings and participates in provincial committees as scheduled by the BCTS.
15.Performs other duties as assigned.
Qualifications:
Education, Training and Experience • Graduation from an approved School of Nursing with current practicing registration as an RN with the British Columbia College of Nurses & Midwives (BCCNM). • Three years recent related experience or an equivalent combination of education, experience and training. Skills and Abilities • Ability to communicate effectively both verbally and in writing. • Skills in autonomous decision-making, problem solving. • Ability to teach. • Ability to participate as a member of a multidisciplinary team. • Physical ability to perform assigned duties.
