Job Summary

Salary Range: $88,990-$127,923/year. The starting salary for this position would be determined with consideration of the successful candidate’s relevant education and experience, and would be in alignment with the provincial compensation reference plan. Salary will be prorated accordingly for part time roles.

 


Job Summary:

As such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Reporting to the Director of Professional Practice, the Collaborative Practice Leader (CPL) works in close partnership with Senior Directors, Clinical Program Managers and Discipline Leaders as a systems leader and change agent, and is responsible for providing leadership, consultation and direction for professional, inter-professional, clinical and academic practice across one or more sites on behalf of an assigned grouping of professional disciplines. This includes liaising with professional discipline clinicians/leaders and other vested parties and representing professional practice contributions in decision making forums. As an active member of senior operational leadership committees, participates in the development and implementation of program and organizational decision making that have operational and/or practice implications.  Additionally, the CPL provides cross agency systems leadership for assigned professional/inter-professional practice priorities and initiatives.



Duties/Accountabilities:

  • In collaboration with Discipline Leaders, ensures professional standards and regulation, practice guidelines and code of ethics for all assigned professions are achieved.  Facilitates education, policy and systems level discussion regarding professional regulation and standards.  
  • Provides leadership in creating, articulating, innovating and sustaining an organization wide vision for inter-professional collaborative practice that emphasizes highest quality, family centered care.  Supports strong inter-professional collaboration through the development of structures, processes and mechanisms that support positive patient outcomes. Utilizes and models effective change management strategies to achieve systems level transformation.
  • Supports organizational strategy deployment, ensuring that professional practice priorities are aligned with organization and provincial strategic priorities.  Tracks, analyzes and reports progress in relation to designated performance indicators.  
  • Provides systems leadership and assignment of appropriate discipline representation for effective profession-specific and inter-professional participation in advancing practice and/or addressing professional practice issues.  
  • Develops and/or guides development with discipline leaders, of systems, mechanisms and structures that support clinicians in exercising accountability and responsibility in the provision of safe, family-centred quality care.
  • In collaboration with relevant discipline leader(s), participates in the review and resolution of interdisciplinary care/clinical issues across the programs and within specific programs/units which may have operational (budget, HHR) and/or practice implications.
  • In collaboration with relevant discipline leader, leads and directs progressive disciplinary plans as required.
  • Represents the professional practice portfolio in decision making forums, including Senior operational forums, for education, research, strategic planning, quality and other defined organizational level initiatives.
  • Collaborates with discipline leaders and other stakeholders in the development of the system wide quality improvement and risk management programs through facilitation of structures, processes and strategies to promote evidenced based practice.
  • Establishes a systems wide framework and process to assess, monitor and evaluate profession specific competency standards and interprofessional competency standards. Directs development and delivery of education programs and other knowledge translation strategies regarding professional standards, regulation and practice as required.  
  • Together with Discipline Leaders and Program Managers ensures that mechanisms are in place to evaluate education funding and education leave request suitability.  
  • Leads and mentors inter-professional dialogue and enhances practice scholarship within and across professional groups.  Collaborates and engages with organizational discipline leaders as required to facilitate planning, innovation and problem solving within professional practice councils and monitors progress toward the achievement of established objectives.  
  • In collaboration with Discipline Leaders, consults on the health human resource development needs.  Participates in organizational strategic planning for the development of a high functioning health human workforce team.
  • In collaboration with Discipline Leaders, PHSA Director of Academic Development and academic partners, participates in strategic planning of academic activity in relation to future HHR needs, current clinical needs, organizational priorities and capacity.
  • Cultivates new opportunities for clinically based research, professional and interprofessional presentation and publications. In collaboration with discipline leaders, participates in the planning, development and evaluation of discipline specific and inter-professional research proposals, projects and surveys and prioritizes according to clinical and resource availability. Facilitates identification of funding/grant opportunities and aligns project-relevant Discipline Leaders together for funding submissions.   Ensures that identified research opportunities are evaluated and ensures that research findings are integrated into practice through policies, procedures, educational programs, and materials.
  • Performs other organizational initiatives and projects as required.

 

Qualifications:

  • A level of education, training, and experience equivalent to a Master’s Degree in a Health Sciences Discipline or related field, plus five (5) years of recent, related experience including two (2) years leadership experience or an equivalent combination of education, training and experience.  
  • Current full registration with own Discipline Professional Regulatory Body.
  • Demonstrated knowledge and experience in facilitation of inter-professional collaborative practice and education.  
  • Current knowledge of regulatory bodies and the Health Professions Act and knowledge of other health care disciplines and their role in client care.  
  • Comprehensive knowledge of organization mission, vision, values, strategic directions and goals.  
  • Knowledge of research process and methodology.
  • Demonstrated skill and ability in designing flexible strategies and to develop project plans and implement new projects by problem solving and leading change.  
  • Demonstrated ability to plan, organize and prioritize work, including the ability to adjust to new or unexpected events.  
  • Demonstrated ability to lead and work collaboratively as a member of an inter-professional team and ability to function as an effective leader, coach, mentor and facilitator.  
  • Demonstrated ability to communicate both verbally and in writing, and to deal effectively with co-workers, physicians, other health care professionals, clients and their families, and external agencies.
  • Demonstrated experience in education planning, development, evaluation and leadership, and a commitment to continuous learning and to participate in and support the learning of others.  
  • Basic computer literacy to operate a computerized patient care information system.  Demonstrated physical ability to perform the duties of the position.
