Job Summary:

The Nurse Clinician, Outpatient Mental Health Services coordinates nursing practise activities such as patient assessment, patient and family teaching and health promotion, acts as an educational resource to the Mental Health Program staff and the community, facilitates and participates in patient conferences, and provides patient care according to established policies, standards, procedures and philosophy at Children’s & Women’s Health Centre of BC. Consults, confers and collaborates with interdisciplinary team members, educational resources and other health care professionals/providers in the community. Acts as a clinical nursing resource and provides clinical leadership on interdisciplinary teams in addressing the health issues of patients. Provides clinical education services to patients and their families, staff, and other health care providers, develops education materials, and conducting education sessions using advanced theory and knowledge.
Duties/Accountabilities:
1. Manages all referrals received by the Outpatient department (OPD) by reviewing, assessing for urgency, prioritizing and making decisions to send referrals to the appropriate specialty clinic in the OPD and/ or redirects referrals to appropriate community resources. Maintains written communication with referral sources and families regarding referral decisions and accessing community based resources. Ensures referrals are tracked in the mental health database.
2. Coordinates and initiates assessments and post assessment referrals by methods such as gathering information from the referral source/client/other relevant persons, responding to patient, family and health care member’s enquiries, evaluating information to identify priority of service required and suitability of admission into program, and liaising with other support diagnostic/health services staff.
3. Acts as an educational resource, and develops teaching plans for patients, families and health care providers in the hospital and community settings.
4. Leads and participates as a member of the health care team in patient case conferences for patient reviews, admissions, discharges and ongoing program evaluation by providing input on a variety of care issues, collaborating with the assessment/triage team, assessing the patient’s needs during the intake procedure to determine patient disposition, and supporting staff with facilitating patient care reviews and conferences. Develops, implements and assesses the effectiveness of individualized care plans for each patient using a relevant nursing model at an advanced level.
5. Identifies, plans and develops educational resources such as pamphlets, web pages for patients and healthcare professionals, teaching videos for patient and family health education, reference care plans, and specialized learning packages for nurses by compiling relevant information, utilizing best practices, determining appropriate form for education resources and material, collaborating with others, and reviewing material to ensure the content is current and cost effective.
6. Acting as a clinical nursing resource, develops individual patient care plans, in conjunction with nursing and medical staff, and other related disciplines in Mental Health, in both the hospital and community setting.
7. Facilitates and enhances communications and team work through methods such as providing information based upon theory/practice and continuing professional education, developing communication linkages, fostering collaborative partnerships and strengthening existing working relationships, resolving clinical issues and advocates for the patients on behalf of the program in collaboration with team members.
8. Liaises with appropriate community and professional groups and agencies.
9. Provides ongoing physical assessment and reviews medication requirements in collaboration with the Mental Health team.
10. Participates in nursing research and other interdisciplinary projects through reviewing current research, collecting data, analysing information and identifying issues/practices which require research, and evaluating the implementation of current theory into practise.
11. Participates in the training of psychiatric and pediatric residents, medical students, and other allied health students in individual and family based crisis mental health assessment, management and discharge planning.
12. Facilitates and participates in ongoing education and in-services to maintain high level of knowledge in the area of outpatient mental health services by methods such as assessing individual/group learning needs; determining resources and learning environment required, developing educational/course material, and delivering educational sessions to staff.
13. Provides input to the Program Director, or delegate, regarding policies, procedures and operational requirements within the program and provides information which addresses patient and family needs and promotes effective use of resources (proposal writing).
14. Provides direction and participates in quality management activities related to the nursing practice within the designated program through assessment, planning, implementation, and evaluation of strategic interventions to improve nursing practice.
15. Performs other related duties as required.
Qualifications:
Education, Training and Experience
• Graduation from an approved School of Nursing with current practicing registration as an RN or RPN with the British Columbia College of Nurses & Midwives (BCCNM).
• Three (3) years recent related experience with working children, adolescents and families with Mental Health disorders, or an equivalent combination of education, training and experience.
• Current CPR and CPI training.

• Ability to operate related equipment including a personnel computer, computerized client care information system and education tools and equipment.
• Physical ability to perform the duties and responsibilities of the position.
