Job Summary


Job Summary:

In accordance with the Purpose, Vision, Values and Coast Salish Teachings, and strategic direction of PHSA, safety, including both patient and employee safety, is a priority and a responsibility shared by everyone at PHSA. As such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Provides nursing services for renal patients and their families.  In collaboration with the nephrologists, assesses health care needs; coordinates professional patient care; implements care programs; provides counselling, teaching and emotional support to patients and their families; coordinates and provides for community outreach; and coordinates (develops, implements and evaluates) educational sessions and material for patients families and community health care professionals.  Serves as a patient advocate, care facilitator and clinical nursing resource.

Duties/Accountabilities:

1.    Serves as an advocate and facilitator for the patients of the program and their families to assist them in obtaining the care and services that they require from the hospital and the community health care and social care systems on a long term basis.

2.    Assesses family learning needs and develops an individualized teaching plan.

3.    Provides education, counselling and support to families of renal patients.

4.    Provides ongoing follow-up education for families with particular attention to care at home and general parental adjustment to child's condition.

5.    Evaluates the level of caregiver's understanding of teaching/training, by interview questioning and return demonstration of techniques.

6.    Assesses, plans and organizes the implementation of discharge planning for all renal patients including: organizing the acquisition and orientation to all required equipment and medications; working with KDS and BCTS to arrange appropriate support f or families; and working with the community health nurses and other community resources as appropriate to ensure a home assessment is complete.

7.    Maintains regular contact with patients on home peritoneal dialysis, assisting them to problem solve problems which arise.

8.    Responds to telephone enquiries from family and community caregivers (family physicians and community health nurse) related to their concerns which have an impact on patient care: treatment protocols/procedures, patient health care concerns, crisis management, and family functional problems. May refer callers to more appropriate resources on the team or in the community as indicated.


9.    Reviews ongoing implementation and effectiveness of treatment plans and patient care procedures by discussion with the patient, and/or professional and family caregivers during hospital stay, clinic visits, or through telephone contacts following discharge.

10.    Provide s nursing care, education and counselling to renal patients in the renal clinic.

11.    Coordinates the development and updating reference care plans and standard care plans for renal patients.

12.    Participates in all Renal Rounds and chairs the noon Renal Rounds.

13.    Documents pertinent information in the hospital and clinic patient charts as appropriate.

14.    Acts as a resource for BCCH staff for nursing care requirements of renal patients.

15.    Acts as a role model to nursing staff, other health care professionals and students.

16.    Acts as a community outreach resource (province wide) by providing general information, resource information and literature related to pediatric renal health care issues, in response to inquiries from families, community health professionals, health care students and other interested persons.

17.    Acts as a resource for nursing students by providing information (verbal and written) about the general treatment associated with renal patients/ families.

18.    Liaises with renal experts in other hospitals and community agencies.

19.    Develops and updates teaching protocols and teaching flow sheets for renal patients.

20.    Develops staff, patient and family educational material and coordinates production with other team members and patient education staff; acquires suitable externally available education material.

21.    Conducts in-service education sessions and informal teaching of nursing and other staff related to renal nursing care. Sessions may occur at BCCH or in the community.

22.    Prepares and presents formal education sessions related to renal nursing care for community agencies and conferences, etc.

23.    Ensures special supplies for dialysis patients are ordered and received in a timely manner.

24.    Identifies the need for capital equipment for renal patients.

25.    Participates in the evaluation and selection of capital equipment for renal patients.

26.    Identifies relevant researchable clinical nursing problems; may develop and conduct research project.

27.    Participates in clinical research projects: provides nursing input into project development, facilitates the implementation of projects by collecting data, coordinating patient involvement and serving as a patient advocate.

28.    Reads literature related to the care of renal patients and maintains a current theory base.

29.    Collects appropriate statistics for renal patients in collaboration with nephrologists and Patient Services Director or delegate.

30.    Participates on nursing and other related communities throughout BCCH and in the community.

31.    Performs other professional related duties as assigned.

Qualifications:

Education, Training and Experience
• Graduation from an approved School of Nursing with current practicing registration as an RN with the British Columbia College of Nurses & Midwives (BCCNM).
• Three (3) years’ recent related clinical nursing experience with pediatric renal patients which includes ambulatory care or community health service experience or an equivalent combination of education, training and experience.
