Job Summary


Job Summary:

In accordance with the Purpose, Vision, Values and Coast Salish Teachings, and strategic direction of PHSA, safety, including both patient and employee safety, is a priority and a responsibility shared by everyone at PHSA. As such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

The Resource Coordinator reports operationally to the Program Leader and for practice general supervision to the Professional Practice Leader, Social Work, and is primarily responsible for providing initial assessments and assisting patients and families in the program area with practical resources and tangible needs. The Resource Coordinator assesses families’ needs, provides information on community resources, supports patient and family education, facilitates referrals, liaises with other support services, and participates in quality improvement, meetings, and planning and carrying out special events.

Duties/Accountabilities:

1.    Assesses patient and family needs for practical supports and assists families in accessing and securing resources internal and external to the hospital.

2.    Assesses families’ needs and provides support services such as transportation, parking, and meal vouchers. Assists families in accessing and completing appropriate forms and applications.  

3.    Provides support with education to patients and families related to their medical and/or psychosocial needs.

4.    Provides community resource information and ongoing support services to patients, families and health care professionals, and facilitates referrals to community resources.

5.    Assists the program staff with managing and communicating information on community resources, and creating systems to update and communicate this information to patients and families.

6.    Participates in quality improvement activities including reporting unsafe situations and reports as per provincial and organization’s policies and procedures.

7.    Participates in the planning and implementation of special events and in the organization of activities for patients and families as needed.

8.    Supports research activities within the program as necessary.

9.    Participates in team meetings and in other meetings relevant to the program.

10.    Performs other related duties as assigned.

Qualifications:

Education, Training and Experience

•    A level of education, training, and experience equivalent to a Bachelor’s degree in an allied health discipline
•    Minimum of one (1) year experience in a health care setting working with a wide range of service providers.

Skills and Abilities

•    Knowledge and experience in family support and pediatrics.
•    Extensive knowledge of community agencies and resources.
•    Ability to liaise effectively with staff at all levels of the organization and with partners in other health centers and community agencies.
•    Broad knowledge of Trauma Informed Care including Collaborative Problem Solving.
•    Excellent interpersonal, oral and written communication skills including training in reflective listening and validation.
•    Ability to work independently with limited supervision.
•    Self-directed and highly motivated with excellent organizational skills.
•    Demonstrated sensitivity, confidentiality, and commitment to family-centered care.  
•    Ability to collaborate effectively within a multidisciplinary care team.
•    Knowledge of Indigenous cultural safety, to provide health care services that are equitable, respectful and safe.
•    Ability to operate related equipment including word processing, spreadsheet and database programs at an intermediate level.
