Family Practitioner, Enhanced Care Housing

The Enhanced Care Housing Team / the Community Trantional Care housing program is a multidisciplinary community team based in Coquitlam that supports forensic review board clients in housing. It provides individualized supports to help clients integrate into community and develop independent living skills. Recovery oriented supports also include illness management skills, substance use supports, as well as supports and skill building related to augmenting social networks, developing healthy habits and routines, assistance with education, finding and maintaining employment. The program also builds relationships with external partners to improve housing opportunities for clients after discharge from the forensic system.

 

Forensic Psychiatric Regional Clinics and Programs are for people with a mental health and/or substance use disorder who have come into contact with the law. The courts may refer them to one of our regional clinics for assessment and treatment. We believe that people, when properly supported, have the ability to manage most aspects of their lives. We focus on people's strengths and resources instead of on symptoms and problems. They are a key member of their treatment team and an active participant in their own care. We help them get the care, support, and treatment they need.

We are seeking a Family Practitioner (FP) for Enhance Care Housing and the Community Transitional Care to help us improve access to primary health care as well as mental health and substance use support for clients to achieve success with reintegration in their community. The FP is an important member of the health care team and working alongside other health care experts on a supportive and inter-professional team, you will provide quality care for clients and help them live an active life during their time in custody and as they reintegrate into the community. This new model of health care provides the opportunity for you to grow your expertise as a Primary Care Practitioner in an environment where you are supported from day one, by leaders and colleagues across disciplines.

 

Be part of a team where you can live and work in your community, positively influence the community reintegration of an individual found NCR-MD or Unfit to Stand trial and see the difference you can make as you serve some of the most complex psychiatric patient population in British Columbia.


Qualifications

 

The work is both interesting and challenging. Experience in working with patients across a broad spectrum of psychiatric illness, substance use disorders, and personality difficulties, is a definite advantage. The interested physicians should be eligible for provisional or full licensure with CPSBC. Certification with the College of Family Physicians, or equivalent, is mandatory. Experience in Correctional Health and/or additional certification or training is an asset.
